Passport

Please scan/submit a copy of the passport information page. The passport must be signed and valid for at least 6 months beyond intended stay.

NOTE: For Non U.S. Citizens Applicant must submit either notarized Copy of Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport). Please contact our office for requirements for non-US Nationals.

Scan/submit standard 2×2 Passport Photo, white background taken within 6 months that is clearly visible with no pixilation.

Letter of guarantee

A Business Letter of Guarantee is required, to be typewritten on your company letterhead and addressed to:

“Consulate of Vietnam / Houston TX”.

The letter must clearly explain the purpose of travel; Guarantee financial support for the visit and return transportation; the letter should be signed by an officer or manager of the company, other than the applicant.

Letter of invite

Please provide a copy of the Letter of Invitation from the company in Vietnam.

Proof of travel & Hotel reservation

Please provide a copy of your flight itinerary and a hotel reservation.

Health record

A copy of the travelers yellow fever is not required for travel at this time.
