Must be signed and valid for at least 6 months beyond intended stay.

NOTE: For Non U.S. Citizens Applicant must submit either notarized Copy of  Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport)

NOTE: If the applicant possess dual citizenship, there will be an additional fee assessed by the consulate

Applications will not be accepted if they are not typed. Please do not leave any blanks, applications that are not fully completed will be rejected by the consulate at the applicants expense.

Photos

Please provide 2 Good Quality Color Passport-Type photographs with white background.

Bank statement

A copy of the applicant’s most recent bank statement it must be three months.

Letter of invite

A letter of invitation from the host who is accepting full immigration responsibility, or if host is an expatriate, a copy of the resident permit should be attached to the letter of invite.

Proof of travel & Hotel reservation

Please provide a copy of your flight itinerary and a hotel reservation.
