Letter of guarantee

A Business Letter of Guarantee is required, to be typewritten on your company letterhead and addressed to: “Consulate of Bolivia / Houston TX”.

Bank statement

Please provide a copy of your bank statement for the last three months or a copy of your credit card, front and back.

Proof of travel & Hotel reservation

Please provide a copy of your flight itinerary and a hotel reservation.

Proof of criminal background

Please provide an original report of your most recent background check. Sam’s passport can assist in obtaining a background check for the City of Houston and most surrounding suburbs.

Health record

Please provide a Color Copy of the applicants most recent yellow fever vaccination certificates/results.
